Cardiac rehabilitation-the road to health for heart patients

Since his coronary artery bypass surgery on October 28 last year, Mr. Jiang, who is not yet 50 years old, has only been left with medication and recuperation, and his original tasty and colorful life has almost disappeared. After the surgery, Mr. Jiang began to try various exercises on his own, such as walking, running, cycling, dumbbells, etc. However, because no scientific assessment was conducted, he was always full of fear and worry when exercising, afraid that the newly bypassed blood vessels would have problems again, and worried about the recurrence of heart disease.

Cardiopulmonary exercise test under the guidance of rehabilitation doctor

On February 15 this year, Mr. Jiang came to our cardiology outpatient clinic with the idea of giving ita try, just in time to be seen by Dr. Gao Lei, who had just received his cardiac rehabilitation training at Fu Wai Hospital. Dr. Gao Lei fully communicated with Mr. Jiang and explained that cardiac rehabilitation would require several months or even longer cooperation between the doctor and patient, and Mr. Jiang strongly agreed to the exercise rehabilitation-based cardiac rehabilitation program and was willing to adhere to the systematic cardiac rehabilitation training.

Mr. Jiang weighed 200 pounds before his cardiac rehabilitation

Gao Lei first performed a cardiopulmonary exercise test on Mr. Jiang to assess his overall cardiopulmonary function, evaluate his exercise risk, exercise capacity and endurance, and develop a cardiopulmonary exercise prescription. After the test, Mr. Jiang's peak load was 195 watts, peak METs 6.4, and peak VO 222.3 ml/min/kg, which was 74% of the expected value, showing mildly reduced exercise capacity, no dynamic changes in ECG during exercise, and appropriate blood pressure response. Combining the resting heart rate and peak heart rate, the target heart rate was calculated, and an exercise prescription was formulated for him. In the first phase, moderate intensity exercise was performed for half an hour of aerobic running exercise on 1, 3 and 5 of each week. After 2 weeks, the patient adapted to this intensity and the duration of aerobic exercise was increased to 40 minutes; after 12 exercise rehabilitation sessions, the cardiorespiratory exercise test was performed again and the exercise prescription was adjusted, and the aerobic exercise was increased to 50 minutes, after which the strength training of upper and lower limbs and core muscle groups was increased; after 12 sessions in this way, the patient entered the third phase. After the cardiopulmonary test was evaluated again, high-intensity interval training and strength training were chosen to be attempted. Since the whole exercise process was conducted under cardiac monitoring, it ensured both the safety and the effectiveness of the patient's exercise. After 36 exercise rehabilitation sessions, Mr. Jiang's weight decreased to 77 kg, BMI 25.4 kg/m2, peak load increased to 230watts, peak METs increased to 9.4, peak VO2 increased to 32.9 ml/min/kg, accounting for 101 of the expected value, and exercise capacity improved significantly, with no dynamic changes in ECG and appropriate blood pressure response during exercise. The entire phase II cardiac rehabilitation not only restored Mr. Jiang's confidence in living a healthy life, but also allowed him to develop good habits of persistent exercise, and even during the home period of the epidemic, he was able to persist in performing appropriate activities of strength without regressing his cardiopulmonary function. Now Mr. Jiang still insists on coming to the clinic three times a week for exercise and exercise, and also actively exchanges ideas with other cardiac rehabilitation patients to encourage them to stick to scientific exercise.

Rehabilitation doctor Gao Lei sets the exercise program

In 2020, the Cardiology Department of Beijing Chuiyangliu Hospital purchased advanced cardiac rehabilitation assessment equipment-cardiopulmonary exercise tester, telemetry exercise ECG system, aerobic exercise equipment running table and bicycle and pneumatic upper and lower limb and core muscle group strength training equipment, and opened a cardiac rehabilitation clinic with a professional medical rehabilitation team to provide cardiac patients with We have a professional medical and nursing rehabilitation team to conduct scientific assessment of cardiopulmonary function, develop personalized exercise rehabilitation plans for each patient, and provide scientific, safe and effective exercise rehabilitation treatment for post-stent and bypass patients with coronary heart disease and high-risk groups with cardiovascular diseases.

I. What is cardiac rehabilitation?

Cardiac rehabilitation is to improve patients' clinical symptoms, enhance their exercise capacity, improve their quality of life, return to normal social life, and prevent the recurrence of cardiovascular disease through five major prescriptions: medicine, exercise, nutrition, psychology, and smoking cessation. Among them, exercise rehabilitation is to give individualized exercise prescriptions through scientific assessment of cardiopulmonary exercise tests, including exercise intensity, exercise time, exercise mode, and exercise frequency, to ensure the cardiac safety and effectiveness of exercise and cardiopulmonary function.

Second, who needs cardiac rehabilitation.

1, coronary heart disease: after myocardial infarction, stable angina, after coronary stent implantation, after coronary artery bypass grafting; 2, after cardiac surgery (prosthetic vessel replacement, heart valve replacement, heart transplantation, etc.); 3, metabolic syndrome: hyperlipidemia, hypertension, diabetes mellitus; 4, chronic heart failure in the stable stage.

Third, what are the benefits of cardiac exercise rehabilitation?

It can increase the contraction force of the heart and improve the function of the heart; 2. Improve the endothelial function of blood vessels and control blood pressure; 3. Improve the peripheral circulation of coronary arteries and achieve the effect of "self-bypass", thus improving myocardial ischemia; 4.promote the synthesis of calcium in bone, delay osteoporosis, and improve physical exercise ability and endurance.
