Sudden cardiac death is a group of dangerous diseases. Epidemiological surveys show that more than 540000 people die from sudden cardiac death in China every year, which is equal to an average of more than 1500 people per day and 1 person per minute.

With the development of medical technology, sudden cardiac death has become a preventable disease for high-risk groups. What can be done to enhance the awareness of sudden cardiac death prevention and control among the population? On August 6, the national tour of the Sudden Cardiac Death Comprehensive Prevention and Control (PSCD) program was held in Guangzhou, and the South+ reporter interviewed experts.

High-risk groups should have ICD implantation as early as possible

The Chinese Medical Association's cardiac electrophysiology and pacing branch director designate, Sun Yat-sen Memorial Hospital, Sun Yat-sen University Professor Wang Jingfeng introduced, at present, for high-risk groups, preventive ICD implantation is the most effective means to prevent sudden cardiac death.

ICD is a device implanted in the body, like a 24-hour "ambulance" for the heart, monitoring the normal heart rhythm every moment, once the ventricular fibrillation, cardiac arrest, ICD can immediately step in and release electric shock defibrillation within 10 to 20 seconds of the arrhythmia, the success rate of defibrillation within this time The success rate is almost 100%.

However, the current prevalence of ICDs is low, with only about 8000 ICD implantations in China each year, a disparity compared to the approximately 540000 people who die from sudden cardiac death each year. The United States, on the other hand, has a far smaller population than China, and yet the annual implantation volume is over 200000 units.

Why is this the case? Wang Jingfeng believes that some physicians themselves do not pay enough attention to it, and many of them do not provide enough education to high-risk groups, so the acceptance of patients is naturally not high.

What are the high-risk groups? Wang Jingfeng introduced, one is a patient with severe heart failure; the second is a patient who has suffered a myocardial infarction; there is another category of patients with hereditary cardiac disease, clinically common dilated cardiomyopathy and hypertrophic obstructive cardiomyopathy, etc.. If someone in the family suffers from one of these diseases, or if someone has suffered sudden cardiac death, they should also be screened at the hospital early. It is important for doctors in the relevant departments to raise awareness, inform them of the risks in a timely manner, guide patients for assessment and screening, and implant ICDs as early as possible if necessary.

On the other hand, the cost of ICDs is also key. At present, ICD equipment is imported, and the cost is generally more than 80000 yuan, which is not a small burden for ordinary families. Huang Congxin, honorary director of the Chinese Medical Association's Cardiac Electrophysiology and Pacing Branch, chairman of the China Atrial Fibrillation Center Alliance, and professor at Wuhan University People's Hospital, called for an appropriate increase in the reimbursement rate for ICDs to help more high-risk groups get a "portable ambulance" in time.

Promote comprehensive prevention and treatment of sudden cardiac death

Sudden cardiac death is usually sudden, and if patients are not rescued within 10 minutes, their chances of survival are very slim. According to a report released by the National Cardiovascular Center, more than 90% of sudden cardiac deaths occur outside the hospital, and the success rate is less than 1%.

Huang said that AED (automated external defibrillator) is a "life-saving" tool for out-of-hospital resuscitation. It is a portable medical device that can diagnose specific arrhythmias and give electric shock defibrillation, and is a medical device that can be used by non-professionals to resuscitate patients in cardiac arrest.

However, at present, China's AEDs are still facing the problem of insufficient number of configurations and low popularity of first aid knowledge. Shenzhen, Guangdong Province, is more prominent in the popularity of AEDs. According to public reports, Shenzhen since the launch of the "public electric defibrillation program" in 2017, more than 14000 AEDs have been installed in public places in the city, and more than 50 people have been successfully treated.

"Shenzhen's approach is a good example. Through the program, we hope to promote the comprehensive prevention and treatment of sudden cardiac death systematically and comprehensively by joining hospitals, the Red Cross and enterprises." Hua Wei, director of the Cardiac Electrophysiology and Pacing Branch of the Chinese Medical Association and professor of Fu Wai Hospital of the Chinese Academy of Medical Sciences, introduced that the academic exchanges at the conference covered in-hospital and out-of-hospital emergency care, ICD and catheter ablation treatment, drug treatment, clinical cases of sudden cardiac death, etc., aiming to popularize related knowledge, comprehensively improve the comprehensive prevention and treatment of sudden cardiac death and reduce the occurrence of sudden cardiac death.
